I agree to participate with Pet Haven’s Adoption Coordinator in taking part in the adoption of my foster animal. The input from fosters is greatly appreciated and taken into consideration for the proper placement of the animal; however, I understand that the final decision remains the sole responsibility of Pet Haven.*

I agree to bring my foster animal(s) to veterinary appointments scheduled by Pet Haven. It is mandatory to bring the foster animal{s) to the scheduled date and location for spay/neuter once the appointment is scheduled.*

I understand that adoptive families are to be accorded respect and privacy. I agree to not engage in excessively casual or aggressive contact with potential or adoptive families.*

If any questions or problems arise, I agree to immediately contact a Pet Haven representative. If the need arises to move the animal(s) to another foster home, the foster must allow Pet Haven a reasonable amount of time to move the animal(s). During the transition, it will be the responsibility of the foster to keep the animal(s) separated and safe at all times.*

I agree to accept all responsibility for any events that may occur as a result of fostering an animal(s). I acknowledge that there are inherent risks with animals, including but not limited to: bites, scratches, bruises, and/or damage to property.*

I agree to hold Pet Haven harmless for any injuries to people, pets or for any loss or damage to property and agree to abstain from any legal recourse against Pet Haven, its agents, officers, employees, volunteers, etc.*

I acknowledge that Pet Haven cannot guarantee foster animals against parasites, diseases, or destructive behavior. I will not hold Pet Haven responsible, nor seek any compensation for damages, medical fees, or other liabilities incurred by my foster pet to myself, my property, or my resident animals.*

It has been recommended to me as a foster that I consult my renters or homeowner's policies to ensure that I am properly covered for injuries to people or animals inside/outside of my residence.*

I understand that foster dogs are not to be be exercised outdoors off-leash in any unfenced area and agree to keep my foster dog on leash when outdoors at all times. (Pet Haven strongly discourages foster dogs from attending off-leash dog parks.)
